Frequently Used Disability Claim Forms


Who the hell wants to scour the internet for forms? I don't.

To help you save some time and effort, we have hosted some of these documents on the site so that you can just click and get them in PDF format.
These forms are the most current versions from the Veterans Benefits Administration.
Feel free to download them, fill them out, and submit them to the Department of Veteran Affairs for your claim.

Application for Disability Compensation and Related Compensation Benefits

For pretty much most of what you might want to claim this form will likely fit the bill as it is the primary claim document used for disability claims.
Some types of claims have additional forms that need to be submitted.

Statement in Support of PTSD Claim
(Combat)

Filling a claim for posttraumatic stress disorder (PTSD)?
This form is used to assist in explaining and providing additional information to help the VBA verify the occurrence of what caused your PTSD.

Note: Form 21-0781 is for COMBAT STRESSORS

Compensation Claim for Individual Unemployability

The VA Form 21-8940 is completed in order to request the benefit of individual unemployability. The form identifies previous stints of employment, which the VBA may reach out to for additional information.

Statement in Support of PTSD Claim
(Personal Trauma)

Have you experienced personal trauma during military service that resulted in posttraumatic stress disorder? These items generally include horrible incidents such as military sexual trauma, vehicle accidents, and other forms of assaults.

Have a smaller family unit with just a wife and child?

Submit this form to tell the VBA who is part of your family so that you are properly compensated!

This is the document that most generalized information can be provided on. The document has required sworn testimony statements and is great for telling how your injuries or disabilities occurred.
